			EI-EZU, Cessna FR172K Hawk XP, Private Incident/Accident Date: 2020-07-11 Report No: 2021-004, Published: 2021-06-10 Image by: Trev Read

	The Cessna FR172K aircraft, with one Pilot and one passenger on board, was en route from Rathcoole Aerodrome, Co. Cork (EIRT) to Tibohine Airfield, Co. Roscommon. When the aircraft was north-west of Loughrea, Co. Galway, the Pilot noticed that the engine oil pressure was decreasing and that some oily smoke had emanated from the engine cowlings. This was followed by the failure of the engine. The Pilot made a MAYDAY call to Shannon Air Traffic Control (ATC), to inform them of the situation and that he was going to carry out an emergency landing. During the attempted landing in an agricultural field, the aircraft contacted the ground hard and sustained substantial damage, resulting in the serious injury of the Pilot and passenger. The Pilot and passenger were assisted from the aircraft by two passers-by. There was no fire.

	The Student Pilot was returning to the departure airfield following a solo general handling exercise. The approach was high and fast, and following the initial touchdown, the aircraft bounced. The aircraft then contacted the runway, nose-wheel first, and following further bounces, was brought to rest on the runway with substantial damage. The Student sustained a minor injury subsequent to exiting the aircraft. There was no fire.

	On 27 May 2020, the paramotor carrying one Pilot departed Magharees, Co. Kerry. The Pilot’s intention was to perform a short leisure flight in the local area. At approximately 12.00 hrs, the paramotor passed overhead a caravan park near the village of Camp and turned to return to the point of departure. The Pilot said that having executed the turn, he did not adjust the setting of the wing trims which resulted in a rapid loss of altitude. The Paramotor struck and became entangled in electrical transmission cables overhead the caravan park. The cables broke and the Pilot and paramotor fell to the ground. The Pilot sustained minor injuries.

	The aircraft, an ATR 72-212A, was approaching Dublin Airport (EIDW) following a scheduled passenger flight from Edinburgh Airport (EGPH) in the United Kingdom. During the intermediate descent prior to commencing the published instrument approach procedure, the aircraft descended below the altitude assigned by the Air Traffic Control Officer (ATCO). The ‘Minimum Safe Altitude Warning’ (MSAW) activated on the ATCO’s radar screen to warn the ATCO about the aircraft’s continued descent. When contacted by the ATCO, the Commander initiated a missed approach, climbed to a safe altitude and obtained radar vectors for a second approach. The aircraft subsequently landed at EIDW without further incident. There were no injuries reported to the Investigation and the aircraft was undamaged.

	The Savannah S aircraft departed from Navan Airfield, Co. Meath for a local flight. Shortly after take-off, and while still over the runway, the aircraft experienced an uncommanded loss of engine power. The Pilot attempted an emergency landing onto the remaining runway. After touchdown, the aircraft passed through a wire fence at the end of the runway and came to rest nine metres beyond the fence. The Pilot, who was the sole occupant, was uninjured and exited the aircraft unaided. The aircraft sustained substantial damage. There was no fire.

	The aircraft, an ICP Ventura, was approaching the easterly runway at Tubride Airfield, Co. Meath. As it was flared for landing, the left wing dropped, and struck the ground. The nose landing gear subsequently impacted with the ground and detached from the aircraft. The aircraft slid along the runway on its nose cowling for approximately 20 metres, resulting in substantial damage to the propeller blades. There were no injuries to either of the two occupants on board. There was no fire.

	The Cessna F172P aircraft, with one Pilot and two passengers on board, had just flown from Weston Airport, Co. Dublin, to Craughwell Airfield, Co. Galway, where the landing on Runway 34 was fast and long. Upon realising that there was a stone wall located just beyond the end of the runway at the airfield boundary, and that the aircraft was not going to stop in the distance remaining, the Pilot applied engine power and took-off again. The aircraft marginally cleared the boundary wall, briefly climbed and travelled over the field immediately beyond the boundary wall. It then lost altitude, drifted left, and struck a tree at the far boundary of the field, before impacting with the ground and coming to rest. The three occupants evacuated the aircraft with some difficulty and the emergency services brought them to hospital for assessment. There was no fire.

	After a take-off from Frankfurt-Hahn Airport, the aircraft Commander called for ‘gear-up’. During gear retraction, the Flight Crew heard an unusual noise. This was followed by the aircraft rolling to the left, and the illumination of the left-hand landing gear red and green indication lights. The Flight Crew completed the ‘GEAR DISAGREE’ checklist and elected to return to Frankfurt-Hahn. The aircraft landed normally, but as a precautionary measure the Airport Rescue and Firefighting Service attended the aircraft. The aircraft taxied to a parking stand where they were informed by Air Traffic Control (ATC) that a small, metallic piece from the aircraft had been found on the runway. Damage to the upper surface of the left wing was observed by the Crew and passengers. No injuries were reported to the Investigation. There was no fire.

	The aircraft, a BRM Land Africa, was on its final approach to Runway (RWY) 25 at Letterkenny Airfield (EILT) when it veered to the left in crosswind conditions prompting the Pilot to attempt a go-around. The left wing struck the ground. The aircraft pivoted about the wing tip and impacted nose first into a boundary fence and ditch on the southern side of RWY 25. The Pilot was uninjured and exited the aircraft unaided. The aircraft sustained substantial damage. There was no fire.

	During an attempted take-off from an agricultural field near Bellharbour, Co. Clare, directional control of the "TrikeBuggy Bullet" paramotor was lost, resulting in it colliding with a dry stone wall at the south-western perimeter of the field. The Pilot, who was the sole occupant, sustained fractures to both feet and to one arm. He was removed from the scene by the emergency services and brought to hospital. There was no fire.
